When a research or a person writes appropriate question, it builds the entire process of researching and determining evidence becomes simple and direct.
Obtaining appropriate outcome helps the process of looking for evidence. PICOT question may appear strange, but this depends on the outcome that an individual is focused to obtain. Applying this format can assist the researcher to find effective evidence in a more rapid and effective manner. Applying such a PICOT format may assist a researcher to develop intended clinical questions and direct his research for evidence and relevant information.
For instance, a researcher may intend to understand the impacts of flu vaccination on the influence of pneumonia affecting older adults. The researcher can fill the blank spaces as illustrated:. In …………. Phow do………….

We acknowledge that many health care professionals prefer the more encompassing terminology of 'Evidence-Based Practice' or 'EBP'. We hope that this guide is helpful to everyone interested in evidence based health care. Several groups have been leaders in Evidence Based Medicine. Their initiatives in promoting the practice of EBM are numerous and are detailed in various sections of this guide. Please feel free to contact us with any questions at lib-pref uic.

Evaluating the performance of the evidence in clinical application. Introduction to EBM Please feel free to view this introductory lecture, which provides history, definitions, and general background information that is helpful when first learning about evidence based medicine. AHRQ's mission is to produce evidence to make health care safer, higher quality, more accessible, equitable, and affordable, and to work within the U. Department of Health and Human Services and with other partners to make sure that the evidence is understood and used.
Centre for Evidence Based Medicine more CEBM aims to develop, teach and promote evidence-based health care through a variety of methods so that all healthcare professionals can maintain the highest standards of medicine.
University of York - Centre for Reviews and Dissemination more A research department that specialises in evidence synthesis, assembling and analysing data from multiple research studies to generate policy relevant research. HIRU's mission is to improve the effectiveness and efficiency of health care by providing innovative evidence-based information products and systems to health professionals, patients, policy makers, and the public.
Joanna Briggs Institute more The JBI considers evidence-based healthcare as decision-making that considers the feasibility, appropriateness, meaningfulness and effectiveness of healthcare practices. The best available evidence, the context in which care is delivered, the individual patient and the professional judgement and expertise of the health professional inform this process.PICOT is an approach for developing a nursing question in a format allowing users to approach different nursing research questions with a benefit of evidence-based practice.
It eliminates the potential to assume various inappropriate and including it in the research as it is likely to skew the results to the extent of making them dysfunctional. PICOT is an acronym standing for the following: P-Population or patient problem: It describes the patient in terms of health status, sex, age and race etc.
I-Intervention: These are the plans for attempting to address the health issues of the patient. These actions include specific tests, medications and therapies. C-Comparison: Compares alternatives to the plan such as a different type of treatment or an alternative to treating a patient. O-Outcome- Stands for expected results after an intervention. It might be reducing symptoms, eliminating symptoms or attaining full health.
T-The 'T' stands for the time it takes for an intervention to achieve the desired outcome or observation of the patients. Time is optional as you can decide to concentrate on PICO only. A PICOT question format idea should be a consistent formula to develop researchable and answerable questions.
The process of finding and evaluating evidence will be more straightforward when you create a good PICOT question. PICOT questions should bring multiple concepts that relate to a research topic or a specific clinical situation together. Qualitative questions: Aim at gaining an understanding or discover the meaning of phenomena. These questions ask about the experience of a person or population regarding certain circumstances or situations. Quantitative questions: these are questions that compare two or more individuals sometimes groups based on different outcomes with an aim at discovering the cause and effect relationships.
The results of quantitative questions are associated with interventions or exposures. Forming a question using the PICO framework requires deeper thinking about the type of question to ask.
It should either focus on therapy, diagnosis, prognosis, etiology or prevention to give the question a particular domain. It will be a time-consuming and difficult venture to identify the most appropriate resources and find relevant evidence without a well-focused question. PICO helps to form a question and facilitate quick and wider literature search. PICO framework helps to identify the main elements of a question and simplify writing a question statement.
These are some examples of PICO questions that fit to the framework. Example Does hand washing by healthcare providers reduce transmission of hospital-acquired infections? C-Alternative to hand washing such as wearing a mask or gloves. O-reducing infection. Do turning bedridden patients reduce the risk of pressure ulcers than pressure mattresses? P-Pressure ulcers in bedridden patients.Turner, M. Supporting your practice: Evidence-Based Medicine. The goals of the tutorial are to clearly outline the theory of EBP and to explain how that theory can be put to practice in the day-to-day work of caring for patients.
The cycle begins again with an assessment of the patient and the patient's care. The tutorial focuses largely on efficient literature searching and therefore on asking questions and acquiring the best evidence. In particular, it suggests specific strategies for finding evidence from primary studies, systematic reviews and meta-analyses using EBP tools.
Appraisal is also touched upon, with suggestions for scanning search results to identify articles more likely to yield robust, applicable evidence. This tutorial assumes you already have some familiarity with basic and advanced PubMed search techniques, as well as with MeSH searching.
There are many excellent on-line and print guides that address the critical appraisal of a research report and the application of evidence to an individual patient's care. You will find links to selected resources that provide especially rich content in these areas in the Appraise and Apply modules of this tutorial.

It is best to work systematically through the various modules, starting with Introduction: About Evidence-Based Practice, and then the five modules. EBP has developed over time to now integrate the best research evidence, clinical expertise, the patient's individual values and circumstances, and the characteristics of the practice in which the health professional works. So, EBP is not only about applying the best research evidence to your decision-making, but also using the experience, skills and training that you have as a health professional and taking into account the patient's situation and values e.
The process of integrating all of this information is known as clinical reasoning. When you consider all of these four elements in a way that allows you to make decisions about the care of a patient, you are engaging in EBP.
EBP is important because it aims to provide the most effective care that is available, with the aim of improving patient outcomes. Patients expect to receive the most effective care based on the best available evidence. EBP promotes an attitude of inquiry in health professionals and starts us thinking about: Why am I doing this in this way?
Is there evidence that can guide me to do this in a more effective way? As health professionals, part of providing a professional service is ensuring that our practice is informed by the best available evidence. EBP also plays a role in ensuring that finite health resources are used wisely and that relevant evidence is considered when decisions are made about funding health services. Before EBP health professionals relied on the advice of more experienced colleagues, often taken at face value, their intuition, and on what they were taught as students. The PICOT question format is a consistent "formula" for developing answerable, researchable questions.
When you write a good one, it makes the rest of the process of finding and evaluating evidence much more straightforward. O: Outcome - risk of disease, accuracy of a diagnosis, rate of occurrence of adverse outcome.
T: Time - the time it takes for the intervention to achieve an outcome or how long participants are observed. Note: Not every question will have an intervention as in a meaning question or time when it is implied in another part of the question component. Melnyk B.
